| QuadMed operates nine rehabilitation facilities staffed by Licensed Physical therapists, Occupational therapists, and Licensed Athletic Trainers who work under the direction of the treating physician to design an individualized rehabilitation program. Our centers' extended hours are designed to accommodate a wide range of shifts and schedules.

Scheduling an Appointment
To schedule an appointment, stop by or call your Rehab center. Refer to the QuadMed Rehab Centers above for site-specific business
hours and phone numbers.
Eligibility
All benefit eligible Quad/Graphics’ employees and their dependents
as well as certain corporate clients of QuadMed, can be treated at the Rehab
centers. All you need to do to be treated is get a written referral
for Rehab from a licensed physician and bring that with you to your first
rehabilitation appointment
Cost
There is a $6 co-pay per visit for current eligible employees and
dependents of Quad/Graphics. Other participating employees should consult
their benefits administrator.
Quad/Gold participants are required to provide a have a $15 dollar co-pay per visit. Contact Quad/Claims for current co-pay information.
In addition to injury rehabilitation, Quad/Rehab services include injury education/prevention, ergonomics programs and Return-to-Work programs.
