Integrated Care Management
Our integrated healthcare management for chronic condition and care management programs is physician-driven and fully integrated across the care continuum. Once an at-risk individual is identified, our goal is to have them seen by a physician, and establish a primary care physician relationship/medical home. This relationship allows our physicians to direct both the immediate treatment and ongoing care for the most effective employee health system. QuadMed physicians interface with our nurse educators/disease managers, as well as dieticians, to ensure the patient has the necessary support once they leave the physician’s office. Via the Electronic Medical Record (EMR), the primary care physician can trigger a dietary or nurse educator/disease manager follow up. Our disease managers have access to the EMR, enabling a fully integrated care management and patient-centered approach.
 
Targeted Conditions
QuadMed has evidence-based programs designed for major conditions, including diabetes, hypertension, asthma and hyperlipidemia. Additionally, we are working to incorporate Medication Therapy Management (MTM), available Q3 of 2011. We target diabetes, hypertension, and hyperlipidemia management, as these conditions comprise the metabolic triad and are core cost drivers of devastating vascular events in working-age adults.
