At QuadMed, protecting the health and safety of our patients remains our highest priority. 

As information about the coronavirus (COVID-19) evolves, we are taking preventive measures to reduce the risk of exposure to our patients while providing the essential health care services you depend on. That’s why we are taking several critical steps:

  • QuadMed MyChart online scheduling services have been temporarily suspended.  
  • Going forward, you must call your health center to schedule an appointment.
  • Because of the precautions we are taking to prevent the spread of the virus, our health center staff will be reaching out to reschedule any patient that has a non-essential appointment, including physicals, wellness exams or other preventive visits.

If you are ill and have the following:

  • fever, cough or shortness of breath OR
  • traveled within 14 days of the start of symptoms OR
  • known exposure to someone with a confirmed case of COVID-19

DO NOT visit the health center. Call first for further instructions.

Please note that scheduling is the only MyChart function that has been disabled. You can still request an e-visit with your primary care provider, request a prescription refill and securely message your provider through your MyChart account. 

QuadMed is actively monitoring the COVID-19 situation and will continue to adapt and update measures to align with recommendations from the Centers for Disease Control and Prevention (CDC) and local health authorities.

Social distancing, self-isolation and hand washing are the best ways to prevent the virus from spreading. For more information on COVID-19, please visit the CDC website.

Login or Sign Up for MyChart

Start by visiting mychart.myquadmedical.com to login or click “Sign Up Now" if you do not have an account. If you received an activation code while visiting the health center or on your patient bill, you can enter that and a few other pieces of information, which will allow MyChart to recognize you. If you do not have an activation code, just click “Don’t have an activation code? Sign up online.” on the right side panel. This method will require you to enter a few more pieces of information about yourself. Once that is complete, you will need to confirm the account setup from the automated email that is sent to the email address linked to your account.

How do I request a copy of my medical records?

To request a copy of your medical records, you must print and fill out the QuadMed Authorization to Release Protected Health Information (PHI) form. You can return the form and submit the request multiple ways.  See how by going to: https://myquadmedical.com/content/medical-records/

What types of health care services are available to me?

Services at each QuadMed location vary per employer.  Please contact your QuadMed health center or reach out to our patient services via email or phone (listed above) to learn more about the services available at your onsite clinic.

How much does it cost to see a provider at a QuadMed?
Co-pays and other services provided by QuadMed are dependent upon your organization’s benefits plan and the selections you have for your individual health care insurance plan.
How do I become a patient at QuadMed?
To be eligible, you must be a member through an employer who is partnered with QuadMed to offer health care services as part of their benefit offerings.  Eligibility varies per employer, with some organizations, this may include spouses and dependent children. Also, depending on the employer, there may be a requirement to be enrolled in your organization’s benefits plan.
Are walk-ins accepted at QuadMed health centers?
This varies per health center, however, to allow this type of flexible scheduling, we do work with our clinics to ensure same-day appointments be made available when possible.  Please login to your MyQuadMed account to schedule an appointment or call Patient Services to inquire about making an appointment.
Is my health information secure?
QuadMed operates in accordance with HIPAA and your personal health information is kept confidential. QuadMed does not share any personal health information with your employer without your permission unless your care resulted from a work-related, occupational health injury. Your data is kept secure, additionally, QuadMed health records cannot be accessed by other members.
